How your information is handled

Sports Therapy Pros collects health information and a photo of your government-issued ID electronically through this intake portal. This notice explains what we collect and how it is protected.

What we collect. Contact details, health history and current concerns you share for treatment planning, your informed consent acknowledgments and electronic signature, and a photo of your driver's license used to verify identity for in-home services.

How it is used. Your information is used only for treatment, identity verification, and scheduling. It is never sold, and it is not disclosed to anyone without your authorization except where the law requires.

How it is stored. Records are transmitted over encrypted connections (HTTPS) and stored in a restricted, access- controlled folder that only Jeff Kirstein, LMT can open. Your ID photo is never saved in your browser; identity images are deleted after verification. This page loads no analytics, advertising pixels, or third-party scripts.

Your access. You may request a copy of your signed intake record at any time and we will provide your electronic record promptly — Texas law allows up to 15 business days from a written request.
